Responsive care begins with understanding your needs and values. That's why our first step is listening. It's how we help you live safely at home with greater independence.
We have developed 12 evidence-based disease management programs covering the most frequently seen conditions including congestive heart failure, cancer, COPD, Alzheimer's disease, and stroke. These programs offer people and their families standardized care with a personal experience.
Our Medicare-certified support is provided through geographically assigned teams for faster response times. We're here for families from Flint to Owasso, Lapeer, Fenton and many communities in between.
Did you know that hospice is fully covered by Medicare? That’s right, you can rest assured that nursing services, medications, supplies and equipment are covered when there is a diagnosis requiring hospice care.
We provide smooth transitions back home and visits from licensed nurses, aides and therapists to help you maintain your quality of life. Our team works with you and your doctor on personalized plans for shorter recoveries and fewer hospital readmissions.
Each day, hospice volunteers touch the lives of our patients and their families. Whether reading to a patient, preparing a meal for a family or cleaning up around the house, each gesture makes a meaningful impact on those we serve.
Whether you volunteer behind the scenes at the office or one-on-one with families, you are a valuable member of our hospice team. All volunteers receive free training at a local office.
