Capturing the Full Story of Care, In Real Time

After years working across both hospital and home health settings,ย Brandon Collins,ย homeย healthย registeredย nurseย with OhioHealth at Home in partnership with Compassus in Columbus, Ohio, knew firsthand how demanding clinical documentation could beโ€”and how often it pulled clinicians away from the very reason they chose the profession in the first place.ย

โ€œI came into home health really looking forย betterย work-life balance,โ€ Brandon said. โ€œBut I started to find myself in the same place Iโ€™d been beforeโ€”finishing visits, going home and still spending hours trying to keep up with documentation.โ€ย ย

Like many clinicians, Brandonโ€™s days were filled with meaningful patientย interactionsโ€”education, conversation and care that extended far beyond checklists and clinical tasks. But capturing that full story on paperย wasnโ€™tย always possible.ย

โ€œThereโ€™s so much that happens during a visit that doesnโ€™t always make it into the documentation,โ€ he said. โ€œNot because itโ€™s notย important,ย butย because youโ€™re balancing time, energy and everything else at the end of the day.โ€ย ย

When AI-supported documentation throughย Nestmedย was introduced, Brandon saw an immediate shiftโ€”not just in efficiency, but in accuracy and representation of care.ย

โ€œIt changed everything for me,โ€ he said. โ€œItโ€™s capturing what Iโ€™m actually doing in real timeโ€”not just the basics, but the full picture of patient interaction.โ€ย ย

Instead of trying to reconstruct each visit hours later, Brandon found that the systemย helpedย surface details that might otherwise be missed.ย โ€œItโ€™s catching the moments where you go above and beyond,โ€ he said. โ€œThe things you donโ€™t always give yourself creditย forย butย that really matter in patient care.โ€ย ย

For Brandon, one of the most important distinctions is that the technologyย doesnโ€™tย replace clinical judgmentโ€”it enhances it.ย โ€œIt gives you a strong starting point, but you still own the documentation,โ€ he said. โ€œYou review it, refine it and make sure it reflects exactly what you meant.โ€ He notes that built-in safeguards encourage clinicians to stay engaged in the process, ensuring both accuracy and accountability.ย

โ€œAt the end of the day, weโ€™re responsible for what goes into that chart,โ€ he said. โ€œThis just helps us get there in a smarter, more efficient way.โ€ย ย

Reclaiming Time and Balanceย

Perhaps theย most meaningful impact has been the shift in how Brandon uses his time.ย

With documentation now supported throughout the day,ย heโ€™sย no longer saving multiple notes for late-night completion.ย โ€œIโ€™m able to use time between visits to actually finish documentation instead of stacking everything at the end of the day,โ€ he said.ย That shift has made a difference not just professionally, but personally.ย โ€œWhen I get home, Iโ€™m not just logging back in for hours,โ€ he said. โ€œIโ€™mย cooking dinner,ย Iโ€™mย with myย kids. Thatย balance matters.โ€ย ย

As documentation requirements continue to evolve, tools likeย Nestmedย are helping bridge the gap between clinical care and clinicalย capture,ย ensuringย the full story of patient interactions is recognized and recorded.ย

For Brandon, the value is clear.ย

โ€œItโ€™s not just about saving time,โ€ he said. โ€œItโ€™sย about making sure the careย weโ€™reย giving is fully seenโ€”and thatย weโ€™reย supported in doing our jobs the wayย theyโ€™reย meant to be doneย
