Medical Record Requests
In accordance with our Privacy Policy, Compassus provides an electronic or paper copy of medical records. Generally, medical records are released to the following individuals pursuant to state and federal law:
- Patients requesting their own records
- Legal guardians or legal representatives of the patients
- Third party requests, such as attorneys or insurance carriers
- Health care providers requesting records for continuity of care
Hospice or Palliative Medical Records
For hospice and palliative medical records, a request for electronic or paper records may be made through ChartRequest* here:

Home Health Medical Records
For home health medical records, a request for electronic or paper records may be submitted by email to HHMedicalRecords@compassus.com*. Your email should include this attached form.
