Medical Record Requests
In accordance with our Privacy Policy, Compassus provides an electronic or paper copy of medical records. Generally, medical records are released to the following individuals pursuant to state and federal law:
- Patients requesting their own records
- Legal guardians or legal representatives of the patients
- Third party requests, such as attorneys or insurance carriers
- Health care providers requesting records for continuity of care
For hospice and palliative medical records in the following states Alabama, Missouri, Texas*, Virginia* and Washington*, a request for electronic or paper records may be made through ChartRequest: Medical Records Request – | ChartRequest
For hospice and palliative medical records in all other states*, a request for electronic or paper records may be made directly to the hospice/ palliative program where services were provided.
For home health medical records, a request for electronic or paper records may be submitted by email to HHMedicalRecords@compassus.com. Your email should include this attached form.
*If you received services at any of the Mercy Health by Compassus locations, and those services ended prior to May 1, 2024, please direct your request to Mercy Health.
If you received services at any of the Bon Secours by Compassus locations, and those services ended prior to May 1, 2024, please direct your request to Bon Secours.
If you received services at any of the OhioHealth with Compassus locations, and those services ended prior to September 3, 2024, please direct your request to OhioHealth.
If you received services at any of the Providence at Home with Compassus locations in Washington or Texas, and those services ended prior to May 1, 2025, please direct your request to Providence Health.
Patient privacy is a priority at Compassus. Please be advised that all requests for medical records are subject to review to ensure compliance with applicable laws. You may be asked to provide additional information to validate a request.
