Hospice eligibility, made clear
Medicare hospice eligibility requires two physicians (the patient’s attending physician, if applicable, and our hospice medical director) to certify a prognosis of 6 months or less if the illness follows its expected course. Eligibility is governed by Local Coverage Determinations (LCDs) and supported by disease-specific guidelines.Â

How a referral works
→ Submit a referral
→ We confirm receipt and respond to most referrals within 2 hours during business hours. Our intake team reviews demographics, payer, diagnosis and clinical summary. Â
→ A clinician evaluates the patient, same day when possible. Our nurse meets with the patient and family at their location to discuss the hospice benefit and complete the eligibility assessment.Â
→ Admission and certification: Once eligibility is confirmed and the patient and family elect the benefit, care begins, often within 24 hours of referral. Â
→ You stay informed. You will receive admission confirmation and ongoing communication on the cadence you prefer.
