Predictive analytics and clinical decision support
Compassus uses predictive analytics, combined with physician oversight, to identify patients who may be approaching the last week of life. This supports timely goals-of-care conversations, more accurate prognostication and better-aligned care.
Continuous home care for crisis symptoms
When pain or other symptoms cannot be managed with routine visits, our continuous home care service provides extended nursing in the home, supporting patients’ goals to remain where they call home.
General Inpatient (GIP) capacity
When symptom management requires inpatient-level care, Compassus coordinates seamless transitions to contracted inpatient settings (GIP availability may vary by market).

Veteran-specific care planning, recognition and trauma-informed end-of-life support for those who served.
Closed-loop communication back to you
Admission confirmation, plan-of-care updates and notification of death are sent to the referring clinician on a cadence you choose.

Compassus delivers an integrated continuum of home-based care: hospice, palliative care, home health, infusion therapy and advanced care management. When a patient’s needs change, we can help you place them at the appropriate level of care.
A complete interdisciplinary team, available 24-7
Every patient receives a registered nurse case manager, hospice medical director oversight, medical social worker, hospice aide, chaplain and bereavement support. Trained volunteers and complementary therapies are available where offered. RN and aide visits are typically scheduled 2 to 3 times per week when consistent with the plan of care. For symptom escalations, end-of-life events or family questions, your patients reach a Compassus clinician, day or night. Calling Compassus first supports clinically appropriate care at home and helps reduce avoidable ED visits and hospital admissions. When something changes at 2 a.m., you reach a Compassus clinician who knows your loved one’s care plan. Many situations can be managed right there over the phone, so your loved one does not have to leave home.

You know the realities of late hospice referrals: shorter lengths of stay, more acute care utilization in the final months of life and families who later say they wish they had known sooner. We share your goal of getting eligible patients to the right care at the right time. 

Compassus has built our intake, evaluation and clinical pathways to make referrals easier for your team and better for your patients. Our interdisciplinary team works as an extension of your care, ensuring timely admissions, clear communication and care plans aligned with patient goals.

Hospice eligibility, made clear

Hospice is covered by Medicare, Medicaid and most private plans. Under the Medicare Hospice Benefit, patients pay nothing for covered services related to their illness, including the care team, medications and equipment.
