Compassus and Ascension Saint Thomas expand home-based care options in Middle Tennessee

BRENTWOOD, Tenn. (October 17, 2023) – Middle Tennessee patients in need of home-based care have a new option. Ascension Saint Thomas Home Health is transitioning to Compassus, a national leader in integrated home-based care. This transition adds home health care to the continuum of home-based care Compassus currently provides the community. Ascension Saint Thomas is committed to building a system of care that meets the needs of Middle Tennesseans throughout their lifecycle and the transition to Compassus will strengthen home-based patient care.    

Compassus provides palliative care and hospice through its program in Columbia, Tennessee, which serves Maury, Hickman and Lewis counties. Ascension Saint Thomas together with Compassus, will administer the home health services formerly provided by Ascension Saint Thomas Home Health in Hickman, Benton, Humphreys, Lewis, Williamson, Maury, Dickson and Perry counties.

Ascension Saint Thomas and Compassus continually evaluate the varying levels of care needs of patients and communities to meet the overwhelming preference of most people to receive care where they live whenever possible. With the population of Tennesseans ages 60 and older expected to increase by more than 300,000 over the next 10 years, additional home-based care options are needed.  

Compassus provides a continuum of home-based services including home health, palliative care and hospice in partnership with health systems like Ascension and other long-term care partners from more than 250 points of care across 29 states. The transition of Ascension Saint Thomas Home Health to Compassus is an example of how hospitals can integrate home-based care services to support staff to deliver a more integrated care experience for patients and their families.

Home health care focuses on preventing, monitoring and managing health conditions where the patient lives. It provides access to support with medications, wound care, pain management and other specialized care to help patients live as independently as possible in their own homes. It also offers smooth transitions home after illness, injury or surgery with visits from nurses and therapists who help patients regain their strength and quality of life. Skilled home health care is available around the clock and can include home visits, phone support from registered nurses, and regular monitoring of vital signs. In-home rehabilitation services may help patients regain mobility, improve balance and coordination, and reduce pain.
