Director of Clinical Services

As Director of Clinical Services, you are the operational and clinical backbone of the hospice program — managing your team, ensuring quality of care and building the culture that makes exceptional hospice possible.

Supervise patient and family care aligned to each individualized plan of care, ensuring appropriateness, continuity and quality.

Lead and develop the IDT -- scheduling, mentoring, 1-on-1s, productivity and performance management.

Oversee the consultative process between the Medical Director and the IDT to coordinate clinical decision-making.

Ensure adherence to state and federal regulatory requirements across clinical documentation and care delivery.

Support EMR documentation workflows and operational efficiency for your clinical team.

Lead IDT meetings and serve as the bridge between clinical operations and organizational leadership.

Qualifications We Are Looking For

We are looking for a clinical leader with proven home-based care experience, strong regulatory knowledge and the personal commitment to deliver compassionate, community-first care.

Active Registered Nurse (RN) licensure required

Minimum of three years of healthcare experience, preferably in hospice, home health or nursing facility settings

Leadership or management experience strongly preferred

Bachelor’s degree strongly preferred; BSN a plus

Experience with Electronic Medical Record systems a strong plus

Strong organizational, interpersonal and communication skills

Valid driver’s license and auto liability insurance
