You are not triaging paperwork. You are identifying patients who need home-based care and making the case for them.
Partner with advanced practice providers, case management and nursing teams to ensure every handoff is accurate, timely and human.

The Clinical Care Partner role is tied directly to outcomes that matter: hospital readmission rates, length of stay, timely initiation of care and referral quality. Required
Preferred
- Active, unencumbered RN, LMSW, LCSWor LICSW licensure
- Associate’s degree in Nursing, Health Sciences or related field
- Current CPR certification
- Compliance with JV hospital partner occupational health requirements
- Bachelor’s degree in Nursing or Health Sciences
- 2 to 3 years in care coordination or discharge planning
- Hospital, home health,hospice or post-acute care experience
- Experience with Epic and referral platforms
