Our Care Delivery Model brings a standardized clinical playbook into every patient encounter and adapts it to the individual patient and family. The result is a more consistent patient experience, more reliable symptom control and a model of care your team can rely on. 

  • Evidence-based clinical pathways for cardiac, pulmonary, dementia, oncology, renal and neurologic patients. 
  • Predictive analytics that combine data with physician oversight to identify patients approaching the last week of life, supporting earlier goals-of-care conversations and reducing late-week-of-life ED utilization. 
  • Comfort kits in the home for fast, effective management of acute symptoms.

Hospice eligibility, made clear

Medicare hospice eligibility requires two physicians (the patient’s attending physician, if applicable, and our hospice medical director) to certify a prognosis of 6 months or less if the illness follows its expected course. Eligibility is governed by Local Coverage Determinations (LCDs) and supported by disease-specific guidelines.

Mercy Health Hospice by Compassus delivers an integrated continuum of home-based care: hospice, palliative care, home health, infusion therapy and advanced care management. When a patient’s needs change, we can help you place them at the appropriate level of care.
A complete interdisciplinary team, available 24-7
Every patient receives a registered nurse case manager, hospice medical director oversight, medical social worker, hospice aide, chaplain and bereavement support. Trained volunteers and complementary therapies are available where offered. RN and aide visits are typically scheduled 2 to 3 times per week when consistent with the plan of care. For symptom escalations, end-of-life events or family questions, your patients reach a Mercy Health Hospice by Compassus clinician, day or night. Calling Mercy Health Hospice by Compassus first supports clinically appropriate care at home and helps reduce avoidable ED visits and hospital admissions. When something changes at 2 a.m., you reach a Mercy Health Hospice by Compassus clinician who knows your loved one’s care plan. Many situations can be managed right there over the phone, so your loved one does not have to leave home.

How a referral works

→ Submit a referral

→ We confirm receipt and respond to most referrals within 2 hours during business hours. Our intake team reviews demographics, payer, diagnosis and clinical summary.  

→  A clinician evaluates the patient, same day when possible. Our nurse meets with the patient and family at their location to discuss the hospice benefit and complete the eligibility assessment. 

→  Admission and certification: once eligibility is confirmed and the patient and family elect the benefit, care begins, often within 24 hours of referral.  

→  You stay informed. You will receive admission confirmation and ongoing communication on the cadence you prefer.
