Medical Record Requests

In accordance with our Privacy Policy, Compassus provides an electronic or paper copy of medical records. Generally, medical records are released to the following individuals pursuant to state and federal law:

Patients requesting their own records
Legal guardians or legal representatives of the patients
Third party requests, such as attorneys or insurance carriers
Health care providers requesting records for continuity of care

For hospice and palliative medical records, a request for electronic or paper records may be made through ChartRequest.

For home health medical records, a request for electronic or paper records may be submitted by email to HHMedicalRecords@compassus.com. Your email should include this attached form.

Patient privacy is a priority at Compassus. Please be advised that all requests for medical records are subject to review to ensure compliance with applicable laws. You may be asked to provide additional information to validate a request.
