Building Your Book of Business
● Open new accounts with physicians, hospitals, skilled nursing facilities and care communities
● Grow share within your existing referral network through consistent, trust-based outreach
● Use market tools to identify opportunities and prioritize your efforts
● Track performance against weekly admissions goals — and own the results
Supporting Patients and Families
● Coordinate closely with our clinical team to ensure fast, smooth patient admission
● Be available when it matters — including evenings and weekends for time-sensitive cases
● Provide clinical education to referral partners on hospice eligibility and care models
● Bring feedback from referral sources back to operations and advocate for service improvements
