RN Registered Nurse (RN)
Bring Your Nursing Expertise Home
As a Registered Nurse with Ascension at Home in Wisconsin, you will carry a meaningful caseload of home health patients — conducting skilled assessments, delivering direct care, educating patients and families and coordinating with the full care team to support each patient’s goals.
What You Will Do
- Conduct comprehensive patient assessments and develop or revise individualized care plans
- Provide skilled nursing services including wound care, medication management and IV therapy
- Educate patients and caregivers on disease management, medication adherence and self-care techniques
- Serve as the primary clinical coordinator across the patient’s home health episode
- Collaborate with physicians, therapists and social workers to support smooth care transitions
- Document clinical findings accurately and efficiently in our electronic health record system
What You Bring
- Current Wisconsin RN license in good standing
- One or more years of clinical nursing experience; home health or community health background a plus
- Valid Wisconsin driver’s license and reliable transportation

SLP Speech-Language Pathologist (SLP)

As a Speech-Language Pathologist with Ascension at Home in Wisconsin, you will assess and treat communication, cognitive and swallowing disorders in patients’ homes. Your every visit will place your clinical expertise in the exact real-life context where it has the greatest impact.
What You Will Do
- Evaluate and treat speech, language, voice, fluency and swallowing disorders in the home environment
- Conduct dysphagia assessments and implement evidence-based swallowing strategies and diet recommendations
- Provide cognitive-communication therapy for patients with dementia, TBI, or post-stroke conditions
- Develop individualized SLP treatment plans with clear, measurable functional goals
- Provide voice therapy and augmentative and alternative communication support as clinically indicated
- Educate family members and caregivers on strategies to support safe swallowing and communication carryover
What You Bring
- Current Wisconsin SLP license; ASHA Certificate of Clinical Competence (CCC-SLP) preferred
- Master’s degree in Speech-Language Pathology from an accredited program
- Valid Wisconsin driver’s license and reliable transportation
- Experience with medically complex adult populations preferred

OT Occupational Therapist (OT)

As an Occupational Therapist with Ascension at Home in Wisconsin, you will help patients rediscover the daily activities that define quality of life. You will work where functional barriers are real and your interventions are immediately applicable — in the home, in the kitchen, in the places that matter. Positions available in both Milwaukee and Neenah.
What You Will Do
- Evaluate patients’ functional abilities across activities of daily living and instrumental activities of daily living
- Develop individualized OT treatment plans targeting meaningful daily function and independence
- Provide cognitive, perceptual, fine motor and sensorimotor rehabilitation
- Recommend adaptive equipment and train patients and caregivers in safe use and home modification
- Assess and address environmental barriers within the home setting
- Document clinical progress and participate in interdisciplinary care planning
What You Bring
- Current Wisconsin OT license in good standing
- Graduate degree in Occupational Therapy from an accredited program
- Valid Wisconsin driver’s license and reliable transportation
- Ability to work independently and collaboratively in a field-based environment

Not necessarily. We welcome clinicians from hospital, outpatient, skilled nursing and other settings. Ascension at Home provides structured onboarding, field mentorship and ongoing support to ensure your transition to home-based care is confident and well-supported.
What is the coverage area for each program?
Our Milwaukee program serves the greater metro area in southeast Wisconsin. Our Neenah program serves the Fox Valley corridor in northeast Wisconsin. Patient territory is managed to support reasonable drive times and your specific territory will be discussed during the interview process.
Can I apply to both the Milwaukee and Neenah programs?
Yes. If you are open to either location, you are welcome to express interest in both programs when you apply. Our recruiting team will discuss the best fit based on your location, availability and goals.
Is mileage reimbursement provided?
Yes. Ascension at Home provides mileage reimbursement for field clinicians traveling between patient homes in both programs.
